For withdrawal all you do need to is fill withdrawal form, sign it and email a scanned copy. They will wire the funds next day. That is how it worked for me.

Since you have an account denominated in CAD, I am guessing you are from Canada. If that is the case, did you maybe use a Canadian IB to open an account with Dukascopy? If you did then you have two people that can answer your question.
