Depends. Trailing stop applies immediately. I set my trailing stop to 150 with I think a 100 pip initial stoploss. When the trade goes into 100 pips profit, the trailing stop takes over.

What do you mean? It sounds like you want to start a 45 pip trailing stop. In that case, just set the trailing stop to 45. It's already built in.
